Nice find! I think there still might be some of these left in rural Ontario but so far the oldest I've seen in the Toronto area are the curved double guy style.

I have seen similar configuration in some of the episode of Emergency! Here are couple shots at 1959 Buick LeSabre in Emergency!, TV Series, 1972-1978, Ep. 2.12 and 1963 Buick Riviera in Emergency!, TV Series, 1972-1978, Ep. 2.04.

I've seen a few styled like this along/near Admiral Blvd in Tulsa, OK. and some other side roads in that area.

I wonder what signals were originally on the pole, I'm guess 8-8-8 heads with a neon pedestrian signal?

My best guess the original heads were an 8-8-8 Econolite Groove-Back on the pole, and a 12-12-12 Econolite Bullseye overhead with an Econolite exposed Neon pedestrian signal. That was standard for So-Cal back in the day. With those signals on it, the mast arm probably dates back to about 1956 or 57.
