Sylvania FR48T12/350BL/VHO/180 Lit! 
Here's my odd Sylvania reflector VHO blacklight lamp lit up. Note the dimmer half, it's where the reflector is. It's shown here lit up on my GE VHO ballast. 

Unfortunately it seems that ballast is a bit dodgy, it drew 0.7A over the rated current (1.2A) and it seemed to heat up quickly and make a crackling sound. O_o I think I should get a new VHO ballast...
