2x 18 watt preheat fixture
It runs two F18T8 lamps. Cover is always removed because it blocks to much light because it is dirty. Probably from the late 1980s, because the ballast is marked for 220 volts, and but mains voltage was changed to 230 volts in 1988 here, but it has a modern polyester capacitor, not the oil insulated kind used in older fixtures.

Is the capacitor original?

I don't know, it looks old but I would expect a paper-in-oil cap here, the ballast is for 2x20 or 1x40 W tubes, so it seems old.
