There's always a 50/50 chance it's either a defective lamp or the ballast probably may have went which is pretty common with these particular GE fixtures.
does this one have a GE logo on the door? And does it have a "GE signature" rims around the edges (its hard to see in the pic) but those designs can help see which generation this fixture is....if its a 1985 generation or a 1993 or so....They made 3 different generations (changing minor designs) The newest ones did not have either the GE signature rim or the GE logo on the door
I like the M-400 r2, It was the first fixture type I ever had in my collection. It has a classic design and has a good lighting distribution, definitely better than the M-400 r3. The M-400 r3 FCO has that stupid line on the light distribution on the ground. M-400 r2 FCO has a pretty good and stable light distribution.
True, that could be it. I don't see why the big three (GE, AEL, Cooper) have to "improve" their origional designs. They can add newer technoledgy without forming a new luminaire shell. What was wrong with the M-400R2/A2 and FCO companions?

The M-400A2 was the power door version of the M-400R2. They kept that bulky design so that the M-400A2 power/modules could be used on older M-400A lights. but yet the plugs on them were different so they're really not interchangeable without cutting off the plug so who knows lol. The M-400A2 FCO had a great distribution. The M-400R2 FCO i heard was sorta sucky...
It depends. With CWA ballasts, a dead or missing lamp will hurt the ballast no matter what type of lamp it is (MV, HPS, MH, PSMH) but other types of ballasts won't get hurt from missing lamps. However, dead, cycling, or missing lamps will hurt the igniters in HPS and PSMH fixtures.
