ITT Model 13
Here is a shot of how this fixture is configured for Mike. When I got it, the PE receptacle was melted along with some of the internal wiring. I had a busted up 113 and used parts from that to repair this fixture. Surprisingly, with the melted receptacle and wiring, the ballast still worked just fine. I can't tell the date of the ballast since it's pretty rusted and really can't make out the numbers but it's working just fine.

Hmm does this have the metal clip for the hinge? This might be the newer version of mine if the ballast bracket is welded to the ballast but really I have no idea. I noticed the housing is entirely different inside though. Mine has fins on the housing on the side where the ballast is mounted (just on one side though, so the fixture is asymmetrical). Looks like most of the other stuff is the same though.
