I have one of these, only in an (older design?) sleeve. I would post a pic but it's at my house in Alaska so I'll have to wait till I'm up there next.

The older design sleeve might also be a bulk pack sleeve. Retail packed lamps have a more colourful sleeve while bulk packed lamps had a simpler sleeve.

That sounds possible...mine is in a nice cardboard sleeve (not cereal-box paperboard like this one) It has the white/black/orange Sylvania design that seems common on more industrial-type lamps...the stuff you might not find in Home depot or Ace Hardware or places like that...
I was actually surprised to see my local HD had anything besides the typical 175w MV and 70w HPS..

Cardboard sleeve = bulk packed lamp.

All retail Sylvania lamps come in boxes....................
