ITT or AE NEMA head.
This one sports a clear BT mercury vapour lamp, with a still decent light output, the last time I've seen the fixture in person. However the refractor isn't attached to the head anymore, the lamp now serves for this purpose LOL.

i've seen Nema's before with the Refractors being supported by the lamp or hanging by one latch..i always figured it was an idiot that threw a rock up there or something. Rolling Eyes

It could be the wind too, I've seen it before too.

The reflector and lamp were both gone as of March 2011 Sad Too bad, the lamp was BT-shaped and was still giving a good light output. I looked for the refractor, but I didn't find it.
